Free COVID TEST KITS
The Federal Government offers free at-home COVID test kits to the public. Households will be able to order four test kits at a time. These kits will typically ship in 7-12 days. You can order kits at COVIDTests.gov.
ProHealth Care is offering COVID-19 vaccinations to protect anyone 12 or older from the virus. You do not need to be a ProHealth patient to schedule an appointment for your first or second dose.
Those ages 12 to 17 require the Pfizer vaccine, and supplies of that vaccine may limit the number of available appointments. Scheduling for those ages 12 to 17 must be done by phone, and they must be accompanied by a parent or guardian at the time of the vaccination appointment.

Call 262-928-5566 to schedule a vaccination appointment for anyone 12 or older. Schedulers are available to assist you Monday through Friday from 9 a.m. to 4 p.m.
If you are 18 or older, you can schedule an appointment for yourself online. Additional appointments will be added as vaccine shipments are confirmed. Check back if you cannot find a day or time that works for you.
They are not able to accommodate requests for a specific vaccine unless you are scheduling an appointment for your second dose.
