Dental Clinic

 We serve Medicaid and uninsured patients 14 and older living in Waukesha County or the Oconomowoc School District, whose incomes do not exceed 250% of the Federal Poverty Level.  Dental services are available by appointment only.

Emergency appointments are available the same day.

Please bring the following items with you:​

Medicaid Insured patients:

  • Photo ID

  • Insurance card-insurance card must be brought to appointment in order to be seen. 

  • Cash or credit/debit card for co-payments. A transaction fee will apply for all credit/debit card transactions.

Uninsured Patients:

  • Photo ID

  • 30 days of pay stubs for you and your spouse

  • Most recent 1040 tax return

  • Cash or credit/debit card for the copay.  A transaction fee will apply for all credit/debit card transactions.

Applying for Medicaid/BadgerCare

You may be eligible for BadgerCare or other Wisconsin health coverage programs. Click below to check your eligibility and apply through the ACCESS Wisconsin website.

Cancellation Policy

Patients who miss a scheduled dental appointment may be unable to schedule another dental appointment for up to one year. Exceptions for emergencies or extenuating circumstances may be considered at management discretion. Please speak with the Dental Clinic Manager if you have any questions regarding this policy.

If you need to cancel an appointment, we ask that you provide at least 24 hours’ notice whenever possible.

 

Failure to notify the Dental Clinic of a cancellation, or failure to attend a scheduled appointment, may be considered a missed appointment.

 

Arriving 10 or more minutes late to an appointment may also be considered a missed appointment.
