Reduced Fee Dental Clinic

Lake Area Free Clinic opened a six-chair Dental Clinic in 2017.  LAFC Dental is the dental home for those in need of access to basic dental, educational and referral services. LAFC Dental serves Medicaid and uninsured adults who live in Waukesha County or the Oconomowoc School District, whose incomes do not exceed 200% of the Federal Poverty Level.  Dental services are available by appointment only. No walk-ins.

  • Must be an adult Waukesha County resident or live in the Oconomowoc School District
  • Patients accepted:
    • uninsured with income at or below 200% of the Federal Poverty Level
    • patients with Medicaid insurance 
  • We do not accept commercial/private insurance

Please bring the following items with you:

Medicaid Insured patients:

  • Photo ID
  • Insurance card-insurance card must be brought to appointment in order to be seen. 
  • Cash or credit/debit card for co-payments. A transaction fee will apply for all credit/debit card transactions. 

Uninsured Patients:

  • Photo ID
  • 30 days of paystubs for you and your spouse
  • Most recent 1040 tax return
  • Cash or credit/debit card for the copay.  A transaction fee will apply for all credit/debit card transactions.

Cancellation Policy

If you miss one appointment you will not be allowed to schedule an appointment for one year.  Please discuss this with dental clinic manager if you have any questions.

If you must cancel an appointment, you are required to give at least 24 hours notice.

Failure to notify the Dental Clinic of cancellation, or failure to attend a scheduled appointment will be considered a missed appointment.

Arriving 10 or more minutes late for an appointment will be considered a missed appointment. 


Appointment fees for uninsured patients are determined by monthly gross income and family size.  Please contact Dental Clinic Coordinator with questions.

Payments must be made at the time of the scheduled appointment in cash or credit/debit card.  A transaction fee will apply for credit/debit transactions.

When you walk into the LAFC Dental Clinic, you will be greeted by our friendly front desk staff. The bilingual (Spanish/English) team will do everything they can to make your visit is as pleasant as possible.  Our highly skilled professionals will make you feel as comfortable as they can and answer any questions that you have.

Our Dentists

Dr. Schwade

Dr. Robert Schwade DDS graduated from Marquette University, School of Dentistry. He was in private practice at Greendale Dental Associates from 1975-2017. In 2016, he joined the LAFC Dental Task Force to determine how to address the lack of access to dental care for low-income adults in Waukesha County. He serves on the LAFC Board of Directors and he is the Dental Director of the Dental Clinic.

Dr. Schwade was approaching retirement and wanted to still be engaged in dentistry. He had seen how the free clinic had such an impact on his wife, (Dr. Kathleen Schwade who volunteers as a mental health provider at LAFC)

Dr. Schwade says, “This is a great facility with a fantastic staff. I can’t wait to come to work every day. This is the best part of my career.”

Dr. Fernandez

Dr. Fernandez is a dentist from Columbia.  He graduated from Universidad Autonoma De Manizales, Columbia.  He did his advanced education in General Dentistry at Marquette University along with his fellowship of Advanced Clinical Care.  He is a member of the ADA, WDA, and the GMDA. Dr. Fernandez works full time at Lake Area Free Clinic and is on staff at the Marquette University Community South Clinic.  His passion is providing comprehensive dental care to all patients. He enjoys spending time with his family, is a salsa music lover, and a soccer enthusiast. 

Our Dental Hygienists


Hi may name is Tina and I am a Dental Hygienist at Lake Area Free Clinic. I have been with the LAFC since we opened in October, 2017. Prior to being a Dental Hygienist I worked as an assistant and front desk. In total I have over 16 years of dental experience. In 2013 I graduated with WCTC with a degree in Dental Hygiene.

I have lived in the Lake Country area my entire life and have been married to my high school sweetheart for seven years. We have three children. In my free time I like to spend time with my family, bike, and go to northern Wisconsin to visit with extended family.


Hi, my name is Pari and I’m a dental hygienist at Lake Area Free Clinic. I started as a volunteer at LAFC and then joined the team part-time. I earned my dental hygiene degree up in Canada and moved to the United States when I got married. I love working with the LAFC team and building my clients’ confidence through healthy smiles.

I am a sports fan and enjoy watching the Packers and Bucks. I like to cook and entertain friends in our home. I am married with two children and we enjoy going camping and taking family trips to see new places.

I graduated from WCTC with a Dental Assisting certificate and now have 18 years experience.  Making patients feel comfortable and at ease is important to me.

I am a mother of two and a grandma of three.  I love spending time with family, time outdoors, gardening, and baking.


Hi my name is Kayla and I am one of the Dental Assistants here at Lake Area Free Clinic. I mostly assist Dr. Fernandez Monday through Thursday. I have been assisting here since June, 2018. I enjoy working with patients and making them feel comfortable while in the chair.

When I am not at work I enjoy spending time with friends, family, and my four cats.


Hi my name is Brenda and I am a Dental Assistant at Lake Area Free Clinic. I have been here since we opened in October, 2017. I was born and raised in Waukesha and attended the Dental Assisting program at WCTC. I have been in the dental field since 2010 and really enjoy being a Dental Assistant.  I care about helping others and love seeing patients' smiles transform. I am continuing my education and hope to become a Dental Hygienist. I am a mother of one daughter and enjoy spending quality time with my family, shopping for cute outfits, and traveling.
